The lighting doesn't match up, and the inside of the human finger would look nothing like that, the human tissue doesn't have that sort of petal structure to it.

This photoshop originated on 4chan's /b/ some 2 years ago, featuring a woman's breast with lots of these instead of nipples. And people were just as freaked out as you, until someone told them it was a photoshop.
Knees has also been photoshopped this way.
